• Please print out the form, fill it out and bring it to 5 Towns Mini Golf and Batting Range for confirmation of availability of date and time of your party. A $25.00 deposit is necessary when booking is confirmed.

 

• Party hours: weekdays 11 a.m. – 7 p.m. and weekends 11 a.m. – 2 p.m.

 

• Parents, please supervise children on a 1:5 ratio (1 adult for every 5 children)  

 

• 10 guest minimum for all parties please!
